The cat isn't being hurt in anyway, all it is is the cat is trying to meow while licking which is why it makes the sound it does.

The cat isn't being tortured- if it didn't like what the owner was doing it would have either attacked them or ran away. The cat looks pretty content to me, those noises means they are probably enjoying having their back scratched or something.
